Job Description
An exciting opportunity has arisen to join our PCN team in Northlocality of Birmingham, covering areas of Erdington and Castle Vale.

We are a dynamic and forward-thinking PCN team, who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

Following the recent changes in the ARRS Scheme to introduce General Practitioners, we are looking to expand our PCN team by seeking to employ a newly qualified General Practitioner to work alongside our practices and PCN staff (must have qualified within the last 2 years). This does not include International Medical Graduates.

As well as general practice staff, our PCN staff group currently also includes Clinical Pharmacist, Physicians Associates, Social Prescribers, Mental Health practitioners, General Practice Assistants, Physiotherapists, and Care Coordinators.

To perform as an autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. Provide expert professional advice to patients, carers, and colleagues and ensure the maintenance of clinical excellence. To be involved in the provision of education and training to trainees/students and other staff. To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct, ensuring the safety of patients and the quality of their care is the main focus.

Summary of Responsibilities

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients across a Primary Care Network of North Birmingham PCN GP Practices, dealing with a diverse range of health needs.

This will include, but is not limited to, surgery and telephone consultations, home visits, administration, complying with the practices contracts (GMS/PMS/enhanced services/DES).

Main Duties & Responsibilities-

Clinical Responsibilities-

  1. In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports, and correspondence in a timely fashion.
  2. Triage patients presenting to the surgery by telephone, online, and in person.
  3. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
  4. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  5. Screening patients for disease risk factors and early signs of illness.
  6. In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education.
  7. Manage long-term conditions and patients with complex problems.
  8. Recording clear and contemporaneous consultation notes to agreed standards.
  9. Collecting data for audit purposes and complying with QOF requirements.
  10. Delivering enhanced services.
  11. Check, manage & process patient test results.
  12. Refer patients to secondary care and/or relevant organisations and internally within the PCN (eg to additional roles members of the PCN team) as required.
  13. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) in line with local and national guidelines.
  14. Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  15. In general, the post-holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basis.
Other Responsibilities within the Organisation-

  1. Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.
  2. A commitment to life-long learning and audit to ensure evidence-based best practice.
  3. Contributing to evaluation/audit and clinical standard setting within the organisation.
  4. Contributing to the summarising of patient records and read-coding patient data.
  5. Attending training and events organised by the Practice or other agencies, where appropriate and suitable and relevant for your role.
  6. Keep up to date with current evidenced-based approaches to patient care and service delivery, in line with NICE and national frameworks.
  7. Contribute to education and training of trainee members of staff and students.
  8. Attending and contributing to practice and PCN meetings.
Confidentiality-

  1. In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
  2. In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Practice staff, and other healthcare workers. They may also have access to information relating to the Practice and PCN as a business organisation. All such information from any source is to be regarded as strictly confidential.
  3. Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice/PCN may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

  1. Using personal security systems within the workplace according to Practice guidelines.

  2. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
  3. Identifying training needs and support needs to be discussed in clinical supervision.
